摘要
摘要目的探讨慢病管理模式对老年高血压患者的影响;方法选取２０１４年１月－２０１４年１２月我社区老年高血压患者１３０例,随机分为对照组同研究组各６５例,对照组患者使用常规药物治疗,研究组患者使用慢病管理模式,对比两组患者的干预结果;结果在两组患者的干预结果方面,研究组患者各项生理指标显著优于对照组,差异有统计学意义(P＜０．０５);结论老年高血压患者应用慢病管理模式有利于改善患者的血压控制状况,提高患者生存质量,应当进一步推广应用.关键词慢病管理模式;老年高血压;影响AbstractObjectiveToinvestigatetheimpactofdiseasemanagementslowlyinelderlypatientswithhypertension;MethodsinJanuary２０１４－December２０１４mycommunityelderlyhypertensivepatients１３０caseswererandomlydividedintotwogroupswith６５caseseachstudygroupandcontrolpatientsusngconＧventionaldrugtherapy,thestudygrouppatientswithchronicdiseasemanagementinterventionresultscomparedtwogroupsofpatients;theresultsoftheintervenＧtionresultsintwogroupsofpatients,thestudygroupweresignificantlyhigherthanthephysiologicalindicatorsinthecontrolgroup,thedifferencewasstatisticssignificance(P＜０．０５);ConclusionelderlyhypertensivepatientswithchronicdiseasemanagementmodelswillhelptoimprovebloodpressurecontrolstatusofpaＧtientsKaneydiwmorpdrsovethequalityoflifeofpatients,shouldbefurtherpopularized．chronicdiseasemanagementmodel;elderlyhypertensive;influence中图分类号R５４文献标识码B文章编号１００８－６３１５(２０１５)１２－１２２４－０２
