  • 简介:Objective:SHR-1210isanewandpromisinganti-PD-1agentforsolidtumors.DuringthephaseIstudyofSHR-1210,weencounteredanovelbutprevalentimmune-relateddermatologictoxicity:reactivecapillaryhemangiomas(RCHs).ThuswetriedtosummarizethefeaturesofRCHsandestimatetheirrelationshipwithtumorresponse.Methods:Thisprospectiveobservationalstudysystematicallyenrolled98patientswithadvancedsolidtumorsfromApril27th,2016toJune8th,2017inthecontextofthephaseIclinicalstudyofSHR-1210.Thisreportfocusedontheskintoxicities.Patientsunderwententireskininspectioneverytwoweekswhiletakingmedication.TheclinicalcourseofRCHswasrecordedandtheirassociationwithtumorresponsewasestimated.Thedatacut-offdatewasNovember15th,2017.Results:Afteramedianfollow-upof242(range,29–567)days,RCHswereobservedin85.7%(84/98)ofpatientsoncutaneous/mucosalsurfaces;84.5%(71/84)oftheRCHswereevaluatedasgrade1adverseevents.Nograde3or4RCHswereobserved.ThetimeofonsetofRCHswasdosedependentandshortestinthe400mg-dosecohort(P<0.001).SpontaneousandcompleteregressionofRCHswasobservedbothduringandaftertreatment.TheobjectiveresponserateoftumorsforpatientswithRCHswas28.9%(24/83).However,noresponderswereobservedamongthepatientswithoutRCHs.Conclusions:RCHswereprevalentbutmanageableduringtreatmentwithSHR-1210.Itmightaddtotheexpandingliteratureregardingimmune-relateddermatologicadverseevents.

  • 简介:Inordertodefinethemortalitycriteriaofplanarianobjectively,acasestudyofDugesiajaponicaexposedto1-octyl-3-methylimidazoliumbromide([C8mim]Br)wasperformedfollowedbyarecoveryculture.Theresultsshowedthatdefiningplanarianmortalityinlightofitsbodydisintegrationwasappropriate.Ifthedisintegratedtissueofaplanarianwasmorethan1/4ofitsbodylength,itwouldcompletelydegenerate.However,aplanarianwouldregeneratethelosttissueandreturntonormalafterafewdays’recoverycultureifthedisintegratedpartwaslessthan1/4ofbodylength.Therefore,weproposetousebodydisintegrationastheendpointofplanarianmortality,i.e.,1/4bodylengthdegeneratedisthecriticalthresholdofmortalityandsurvivalofplanarianswhenexposedtotoxicants.Thiscriterioncouldbeadaptedinthestandardizationoftestingprotocolsandcomparabilityofacutetoxicitytestorothertoxicologicalresearchusingplanarianasthetestanimal.

  • 简介:摘要BackgroundIt is important to prevent the development of delayed neuropsychiatric sequelae (DNS) in acute carbon monoxide (CO) intoxication, but no effective treatment has been clearly identified. Hyperbaric oxygen (HBO) therapy is one of the treatment options in acute CO poisoning; however, whether it can prevent the development of DNS is controversial.ObjectiveThe purpose of this study is to compare the effectiveness of normobaric oxygen (NBO) and HBO in preventing DNS.MethodsThis prospective observational study was conducted on all patients with CO poisoning admitted to the emergency department of a tertiary hospital from 2016 to 2019. We followed-up patients to determine whether symptoms of DNS occurred at ≤6 months. We matched the propensity score to an equivalent distribution of potential covariates.ResultsA total of 224 patients with CO poisoning were enrolled in this study. NBO was used for 26 patients and HBO for 198 patients. DNS occurred in 40 patients. There were significant differences between the NBO and HBO groups in terms of carboxyhemoglobin, loss of consciousness, dizziness, chest pain, hospitalization, and length of hospital stay. The incidence of DNS was 19.2% in the HBO group, which was higher than the 7.7% observed in the NBO group, but the difference was not significant (P=0.18). After propensity score matching, the incidence of DNS did not differ between the NBO and HBO groups (8.3% vs. 10.4%, P>0.99).ConclusionThere was no difference in the incidence of DNS between groups receiving HBO and NBO in acute CO intoxication.

  • 简介:Capsaicin(trans-8-methyl-N-vanillyl-6-nonenamide)isthemaincomponentinhotpeppers,includingredchilipeppers,alapenos,andhabanero,belongingtothegenusCapsicum.Capsaicinisapotentantioxidantthatinterfereswithfreeradicalactivities.Inthepresentstudy,thepossibleprotectiveeffectofcapsaicinwasstudiedagainstmethylmethanesulphonate(MMS)inducedtoxicityinthirdinstarlarvaeoftransgenicDrosophilamelanogaster(hsp70-lacZ)Bg~9.ThethirdinstarwasallowedtofeedonthediethavingdifferentdosesofcapsaicinandMMSseparatelyandincombination.TheresultssuggestedthattheexposureofthirdinstarlarvaetothediethavingMMSaloneshowedsignificanthsp70expressionaswellastissueDNAandoxidativedamage,whereasthelarvaefeedonthediethavingMMSandcapsaicinshowedadecreaseinthetoxiceffectsfor48-hofexposure.Inconclusion,capsaicinshowedadose-dependentdecreaseinthetoxiceffectsinducedbyMMSinthethirdinstarlarvaeoftransgenicDrosophilamelanogaster.

  • 简介:AbstractTo compare efficacy and safety of nab-paclitaxel plus gemcitabine (AG) with nab-paclitaxel plus S-1 (AS) as first-line treatment for metastatic pancreatic cancer, we conducted a retrospective analysis by reviewing medical records of 53 metastatic pancreatic cancer patients in our institution. They received either AG (nab-paclitaxel 125 mg/m2 on days 1, 8 and gemcitabine 1000 mg/m2 on days 1, 8) or AS (nab-paclitaxel 125 mg/m2 on days 1, 8 and S-1 80-120 mg on days 1-14) chemotherapy. We found that AS had higher objective response rate (36% vs 21.4%), better disease control rate (84% vs 75%), prolonged time to progression (TTP, 7.1 vs 5 months), and improved overall survival (OS, 15.3 vs 12 months) when compared with AG. In Cox proportional hazards model, sex was significantly associated with TTP (P value=.031) and metastatic sites plus treatment after progression were significantly associated with OS (P value=.028 and .01, respectively). The incidence rate of chemotherapy-related adverse events was similar in both groups. Neutropenia (50% and 60%, all grade; 21.4% and 36%, grade 3 or 4, in AG and AS group) and sensory neuropathy (21.4% and 24%, all grade; 3.6% and 4%, grade 3 or 4, in AG and AS group) were the most common hematologic and non-hematologic toxicity. Thus, we believed that AS is a reasonable and convenient alternative for patients treated with AG as first-line chemotherapy for metastatic pancreatic cancer.
